Community Nursing Service
Community Nursing
The service delivers care and support, largely within a domiciliary setting, for people with a wide range of nursing needs, long term conditions, end of life care needs and those requiring specialist nursing interventions such as IV therapy in community settings.
The service works closely with Primary Care, Social Care, local hospitals, mental health services, specialist nurses, therapists, and voluntary sector colleagues to help people to manage their conditions and maintain their independence at their preferred place of care.
The service is available 24 hours a day 7 days a week
Referrals will only be accepted via local triage desks if the patient is already on the community nursing or community matron caseload.

Out of Hours Urgent Care Nursing Service
This service covers the hours of 7pm to 7am.
The team currently covers 12 hours a day across Reading, Wokingham and Newbury with the added area of East Berkshire from 11pm to 7am.
Out of Hour Urgent Care Nursing Service provide Nursing care with the aim of improving quality of care and reducing unnecessary hospital admissions.
Our team have the clinical skills and knowledge needed to attend urgent, unplanned nursing visits, whilst ensuring that patients get the optimal level of holistic care necessary for their condition, in the most appropriate setting and by the most appropriate person.
