Falls Prevention Service
The aim of this service is to reduce the number of falls and resultant injuries including fractured limbs and soft tissue damage, through evidence-based programmes of assessment and education.
The service is available 8.30am to 4.30pm, Monday to Friday.
Referral via the Hub
Community Physiotherapy Service
This service provides physiotherapy when patients are unable to access outpatient services or when assessment and treatment is more appropriate in the home.
Assessment, advice, mobility equipment and treatment are offered for a wide range conditions.
The service is available 8.30am to 4.30pm, Monday to Friday.
Referral via the Hub
Community Reablement Service
This service can be provided when a person is discharged from hospital. Reablement Services provide support for a short time to help individuals recover and increase independence or review ongoing care needs.
The support is provided by a team of people, who discuss and identify realistic therapy aims to work towards, during the individuals time with the service. This is presently a 6 day a week service.
