Heart Function team receive ‘highly commended’ at HSJ Partnership Awards 2025

We’re delighted to reveal that our work delivering the innovative Managing Heart Failure @home project has been ‘Highly Commended’ in the ‘Best Consultancy Partnership with the NHS’ category at the Health Service Journal (HSJ) Partnership Awards 2025.

The project involved using remote monitoring technology to help patients with heart failure manage their condition at home and was developed in collaboration with Frimley Health and Care Integrated Care System, NHS England and Kaleidoscope Health and Care.

“We’re proud and honoured to receive this award from HSJ” said Heart Function Nurse Specialist, Lucy Girdler-Heald.

“Remote monitoring has made a huge difference to the quality of life for heart failure patients."

A remote approach minimises hospital visits, allowing Lucy’s team to increase titration, when medication is increased gradually for each patient, more efficiently. This not only saves clinician and patient time, but allows patients to improve faster.

“The system allows us to detect any problems or deterioration earlier and enables patients to self-care by ‘knowing their numbers’ – for example blood pressure and pulse rate so they can seek help when needed.

“This way of working is now firmly part of our everyday practice so more patients can benefit from remote monitoring and receive care closer to home.”

The HSJ Partnership Awards recognise the vital partnerships between the NHS and suppliers of goods and services.
