If you’ve been diagnosed with a long term condition or are frail, we can make sure that you get access to treatment as quickly as possible to avoid complications and unnecessary visits to hospital.

How we can help you

Once you’re referred into our service, we’ll contact you to book an appointment.

We’re based at St Mark’s Hospital in Maidenhead, and Upton Hospital in Slough.

Once we’ve completed your assessment, we’ll provide you with a personalised care plan to make sure you’re getting the right healthcare, some self-care information and any relevant patient and carer information leaflets.

Our team of nurses, specialists, consultants, physiotherapists, and occupational therapists, will work closely with your other care support groups to offer the right support package for you.

Accessing our service

To access our service, you need to be referred by your GP, the ambulance service or any other healthcare professional.

Unfortunately, we’re not able to accept self-referrals.
