Intermediate Care

When you’re referred to us, we’ll arrange an assessment in your home.

We will talk with you about your goals, and a treatment plan to help you reach them. 

You may need further care calls provided by Local Authorities, alongside our therapeutic intervention.

All our services are time limited and focus on short-term realistic rehabilitation goals.

If we determine that you have ongoing needs, we will care options with you which may include a referral to other appropriate services to help you in the long-term.

Our team

Our clinical team is made up of:

• Occupational Therapists
• Physiotherapists
• Therapy Assistants and

 Depending on your needs and referral, we will decide which team members will be involved in your care.

Depending on your needs there are a range of interventions we can put in place following your assessment to promote your safety and independence in your own home.

These can include:

• Providing equipment to help you complete everyday activities
• Supporting you to regain skills and function
• Provide targeted strength and balance exercise programmes
• Identifying your goals and a plan for next steps

Our Intermediate Care service is available to anyone over 18 and registered with a GP in Reading, West Berkshire, or Wokingham.

You can be referred into our service by your GP or healthcare professional through our Integrated Hub.
