Occupational Therapy

We can support you through many health conditions (such as physical or mental illness, learning disabilities, long term health conditions or the effects of ageing) to become more independent and carry out daily activities.

Once you’re referred into our service, we’ll work with you and other healthcare teams to help you become more independent and carry on with daily activities, hobbies, jobs and relationships.

The teams we work with include:

  • Community Hospitals
  • Learning Disability service
  • Multiple Sclerosis
  • Stroke (Earlier Supported Discharge Service)
  • Neuro-rehabilitation
  • Mobility service
  • Inpatient Mental Health
  • Community Mental Health Team (adult)
  • Older People's Mental Health service 

Children, Young People and Families (CYPF) service

We also work closely with our CYPF Occupational Therapy teams.

Intermediate Care Teams 

Our Intermediate Care Teams can help you recover from your condition in your own home.

You can refer yourself to an occupational therapist by contacting your local council's social care department. 

You can also speak to your GP, nurse, social worker, or other health or social care professional to refer you.

If you're currently being supported by our teams, you can contact them throughout the week, from 9am to 4pm.

Please do not contact our teams if you haven't been referred to us yet.
