East Berkshire Community Physiotherapy
We offer physiotherapy support in your own home, if you're housebound and unable to leave for any appointments.
We will work with you to create the right rehabilitation plan for you, with personalised goals to achieve in four sessions, and help set you up to self-manage long term.
These could include helping you get back to walking and exercising more, and greater independence with your daily living and activities in your local community.
We can help you to manage a range of conditions such as:
- Musculoskeletal injuries e.g. back pain, arthritis
- Orthopaedic e.g. joint replacements, fractures
- Difficulties associated with older age e.g. Falls, reduced balance / mobility
- Neurological conditions e.g. Stroke, MS, Parkinson’s
Depending on your needs, we may offer a range of support, such as:
- Comprehensive assessment to help identify your problems / goals
- Personalised rehabilitation programme in your own home or local environment, to be completed on your own or with support from your carers, family or friends
- One-on-one sessions with an experienced Physiotherapists and/or Therapy assistants
- Pain management advice
- Referral to other appropriate services
- Practice mobility/ functional tasks
- Assessments and possible loan /provision of NHS equipment (dependant on need)
We can provide guides on how to manage your condition, and guide you to other services if you need further specialist support.
We encourage family and carers to be involved, so that you can feel confident that those closest to you are ready to support you.
Our service is available for adults aged 18 and over, who live in East Berkshire.
You must also be registered with an East Berkshire GP.
Referrals can only be made by healthcare professionals or by your GP.
Unfortunately, we do not currently accept self-referrals.
We're available for contact between 8am to 4pm, Monday to Friday.
