Intensive Community Rehabilitation (ICR)
We offer comprehensive rehabilitation for up to two-weeks, as you return to your community as part of your recovery.
We focus on treatments to help you develop the skills and social interaction you need for better overall quality of life
- Develop skills and confidence for your daily activities, to keep your independence
- Personalised interventions, for better overall wellbeing
- Community integration, through active participation in your community
- Comprehensive support, covering your physical, emotional, and social needs
We can help you at home in your rehabilitation in four areas, including assessment, treatment, referral to other service, support guides.
Assessment
We offer comprehensive evaluations to better understand your individual needs, and develop personal rehabilitation plans to suit you.
Treatment
Depending on your needs, we may offer a range of specialised rehabilitation treatment. This includes physical, occupational, nursing designed to aid recovery and enhancing daily functioning:
- Support with personal hygiene
- Assistance with meal preparation
- Customised individual exercise programs
- Nursing care, such as wound management
- Providing necessary equipment
- One-on-one sessions with experienced MTAs for exercise guidance
Referral to Other Services
We can guide you to additional support services such as mental health, social services, or specialised medical care if needed.
Support Guides
We can provide educational materials and resources to help you and your family better understand and manage your situation.
Our service is available only to adults aged 18 and over, who require intensive and short term intervention to help adjust to life back at home.
You must also be registered with a GP in Slough, Windsor, or Maidenhead.
Referrals can only be made by healthcare professionals
Unfortunately we can’t accept self-referrals.
Contact our intake team to begin the referral process. Each referral is triaged to determine the suitability for the program.
